Emergency Identification Tag for Children in Car Seats

Above is an example of a emergency contact identification tag for your child. This card and plastic pouch which is provided by Bonita Springs Fire-rescue is intended to be attached to your child’s restraint devise in your vehicle. The information you provide on this tag can be used by first responders at an emergency scene or vehicle collision. If the adults in the vehicle are unresponsive, the information on the tag can be used to identify the child and contact an alternate caretaker.

 

The information on this card should be written in pencil and updated every six months as should any picture you include in the plastic pouch. Attach the tag to the restraint devise and tuck the tag between the vehicle’s seat and the child restraint devise so no yellow is showing. Predators won’t notice the information tag. In addition, only include the relationship to the child not the alternative caregivers name. This will prevent predators from finding your home address in the phone book. When a first responder arrives to a vehicle collision, the child can remain in their restraint devise and can be removed while secured in the restraint devise. The information tag will be obvious to the first responder when the restraint devise is removed from the vehicle. The picture will enable the first responder to verify that the information matches to the child.

 

Once your child has outgrown their restraint devise, attach this tag to their new restraint devise. If your child has outgrown a booster and is sitting in a regular vehicle seat, place this tag in your glove box.
